A Chair-Side Read For Fitzpatrick 5 & 6

Your skin isn’t difficult. It’s reactive — and the usual
scale doesn’t measure that. How I read deep skin before I ever touch it. The scale most of us were handed — Fitzpatrick — was built in 1975 to predict one thing: how fair skin sunburns, so doctors could dose UV therapy. Types V and VI were
added later, almost as an afterthought. It measures how you burn — not how you mark. That gap isn’t my discovery. Dermatology has pointed it out for years: the burn scale conflates color with reactivity, and for deep skin those aren’t the same thing. What I do with that
knowledge is the part I’ll put my name to — I read deep skin by how it pigments under stress,because in V and VI, that’s the response that actually decides how a treatment goes.
W H E R E THIS COMES FROM—-PLAINLY
I didn’t invent this idea, and I won’t pretend to. Researchers already built better tools for pigment than the
burn scale: the Roberts classification grades a skin’s tendency to hyperpigment and scar, and the Taylor
scale grades how much pigment is already present. I stand on that work. Think of it like a recipe I’ve
adapted — the science is established; I changed a few things for the chair and the conversation, and I put
my name on my version.
W H Y A PIGMENT READ AND NOT THE BURN SCALE
Fitzpatrick ranks burn-propensity, because for fair skin the cost of overdoing it is a burn. For deep
skin the cost isn’t a burn — it’s a mark. Same logic, different endpoint. A scale that only asks “do you burn?” simply can’t see the thing your skin will actually do. So it shouldn’t be the tool that sets how I treat you.

This is well documented: in deeper skin, inflammation can drive pigment down past the surface into the dermis,
where it’s slow to clear — months, sometimes years. There’s no quick “burn” that fades in a week. That’s why the
gentleness isn’t fussiness. It’s the whole game.
This is physics, not punishment. Your skin doesn’t hate you — it’s protecting you, exactly as it was built to.
The science is clear that this pigment response is adaptive — melanin absorbs UV and mops up the damage of inflammation. Deep skin simply has more of that protective machinery, more ready to fire. So when it’s injured, it does precisely what it’s designed to do: it makes pigment. The mark isn’t your skin failing. It’s your skin’s armor responding to an insult. Which means the answer was never “fix your skin” — it’s stop provoking armor this responsive, and work with it
gently. Predictable is good news. Predictable is controllable.

One honest note: tone hints at the range, but I confirm reactivity in the chair — your history and a small test patch —
never from color alone. That’s the whole point: deep skin deserves to be read by how it responds.
YOUR SUNSCREEN IS PROBABLY MISSING HALF THE THREAT
VISIBLE LIGHT FOR V & VI
Why deep skin needs more than UV protection
Roughly half of sunlight is visible light, and pigment cells in deeper skin carry a blue-light sensor (opsin- 3) that switches on the pigment machinery — and keeps it running after the light is gone. That sustained response is largely a deeper-skin phenomenon, which is why visible-light marking shows up from Fitzpatrick III and up. Ordinary UV sunscreen — and clear or white mineral — doesn’t block it.
Choose a tinted mineral sunscreen with iron oxides — iron oxides are what actually block visible light.
Look for zinc oxide + iron oxides on the label. A clear or white formula almost certainly lacks enough.
The tint is the protection — and it solves the white-cast problem, so you’ll actually wear it.
Daylight through windows carries visible light and UV A right through the glass — so a desk by a sunny window counts, even indoors.
A morning antioxidant — vitamin C, and niacinamide also has evidence here — adds support alongside,
not instead of, your sunscreen.
And a myth I’ll put to rest: the sun is the threat, not your phone. Blue light from screens is a small fraction of
the sun’s dose — too low, on its own, to drive the pigment, and short-term screen use hasn’t been shown to
worsen it. Protect against sunlight and daylight. Don’t lose sleep over your laptop.
If you’ve spent years being told your skin was “difficult,” I’d gently offer a different word: responsive. Read it by what it actually does, protect it from the right light, treat it with the right
hand — and it does beautifully. That’s the work I do.
